"Secondary" Restless
Leg Symptoms:
"Primary" (idiopathic) Restless Leg Syndrome
(RLS) is thought to be a brain centered problem involving central nervous system
iron stores and iron metabolism. The only effective treatments discovered to
date are the dopaminergic agents, such as Sinemet, and dopamine agonist drugs
such as Requip, Mirapex, Parlodel, Permax and some of the sedative drugs.
"Secondary"
Restless Leg Symptoms: Many of the symptoms of RLS are also experienced by
patients with venous insufficiency, including itching, tingling, crawling
sensations, and restlessness, a compelling urge to move the legs to relieve
uncomfortable sensations. The only pump for venous blood is movement of the leg
muscles. At night, the brain sends messages to the legs to keep moving to pump
out the edema fluid that has accumulated all day. Extracellular, 'edema' fluid
returns to the vascular system. Some suggest that this edema, movement and
return of extracellular fluids causes "secondary" restless leg
symptoms and that more than 85% of patients who have both RLS symptoms and
venous valve failure are dramatically helped when veins with failed valves are
closed.
Certainly, venous insufficiency causing pain should be treated. These
patients greatly benefit from the improved vascular hemodynamics that results
from treatment and have relief of their pain. These patients also often have
relief of what they have interpreted as 'restless leg' symptoms.
