Sclerotherapy Before and After
Sclerotherapy: Spider and Reticular Veins:
Safe, Only Minor
Discomfort, Very Effective

We first perform an ultrasound evaluation. When there are no valve
problems above the spider veins, we can then proceed with injection of an FDA
approved sclerosant (not saline!) into these small vessels. Two to three
treatments of each leg over six months is generally sufficient. By spreading
these treatments out, fewer treatments are required and each is much more
effective. This is a safe procedure and involves only very tiny and superficial
needle sticks.
We have found that following approach to spider veins is safe and very
effective:
- We first carefully examine both legs by ultrasound for small and greater
saphenous vein, vein of giacomini and anterior and posterior accessory venous
insufficiency. We do his exam to avoid the increased incidence of staining,
matting and recurrence that occurs when underlying conditions are ignored.

- We perform a halogen light exam for blue, feeder
veins associated with clusters of spider veins and inject these veins on the
first treatment visit
.this very much improves the treatment results and
decreases recurrence incidence.
- We use a gentle solution of 0.125% FDA approved Sotradecol -

thus avoiding the pain and scarring seen with outdated use of hypertonic saline
solutions.
- We apply a 2% hydrocortisone cream after each treatment to reduce itching
discomfort.
- We apply 20-30mm HG compression hose, a sheer stocking that you can shower
with, to use for three days following treatment
.this further improves
results.
- We advise patients to avoid sun bathing for two weeks after each treatment.
- We wait 2-3 months for repeat injections, to give the body time to reabsorb
the treated veins and avoid unnecessary treatments.
Costs:
- Ultrasound study: $320
- Spider vein treatment: $200 for each treatment visit. We ask people plan
on six treatments (3 treatments each leg) but often we can include both legs in
one treatment. Therefore complete treatment will take 4 to 6 treatment visits
and cost $800 to $1200. Spider vein treatment is considered cosmetic
and rarely reimbursed by insurance.
- Reticular vein (dark purple, larger than spider veins) treatments: Same
as spider veins but are usually considered medically necessary to
prevent recurrent bleeding episodes and are therefore paid by Medicare and
reimbursed by most insurances.
