1. Laser or
Radiofrequency Endovenous Closure:
These are the "state of the
art" treatments. If a major vein trunk draining the surface of the leg has
backward flow down the legs, we simply close it. We can do this because, after
closing the vein, the blood goes through the many 'perforator' vein shortcuts to
the deep vein systems of the leg. This procedure improves flow to the deep
circulation, decreasing the risk for clots. And, the pressure problems are
removed from the superficial circulation, greatly improving the patient's
symptoms.
Vein closure procedures involve starting an IV in the vein,
gently threading a laser or radiofrequency fiber into the vein, surrounding the
vein with fluid for safety, and then withdrawing the fiber over several minutes.
A band-aid is placed over the puncture. This entire procedure is painless and
takes only about 45 minutes. The patient is then encouraged to take a 15 minute
walk and may return to work and normal activity the same day!
2. Micro-Phlebectomy of Varicose Veins:
Varicose
veins shrink after we close damaged vein trunks above them. If the varicosities
have been too stretched over time to completely disappear, we numb the skin,
make needle punctures about every inch over the vein and remove the vein through
these needle punctures, using a very fine instrument. No cutting. No sutures. No
'downtime'. Only a simple bandage for three days is required.

Sclerotherapy: Spider & Reticular Veins:
We first perform an
ultrasound evaluation. When there are no valve problems above the spider veins,
we can then proceed with injection of FDA approved sclerosant into these small
vessels. Three treatments of each leg over six months are generally sufficient.
By first screening and treating for valve problems, we greatly reduce side
effects and recurrence. By spreading these treatments out, fewer treatments are
required, and each is much more effective. This is a safe procedure and
involves only very tiny, superficial needle sticks.
